What is CoolSculpting, and how does it work?

CoolSculpting is an FDA-cleared, non-surgical, safe, and effective procedure to remove fat, shape, and contour the body. It can be considered a non-invasive alternative to Liposuction as the process removes fat and reduces the physical dimension of the area being treated.CoolSculpting works through the proven scientific principle of Cryolipolysis, where cold temperatures selectively affect fat cells and mark them for removal from the body without damaging surrounding muscle, skin, and other tissue. The original research and discoveries in Cryolipolysis were made by Harvard University scientists Dieter Manstein, MD, and R. Rox Anderson, MD. The process has been further developed and commercialized by Zeltiq Aesthetics, Inc., a medical technologies company based in California. Fat cells in the human body are damaged and eventually die if they are subjected to cold temperatures for a precise amount of time. The cold temperature required the fat cells to be damaged higher than temperatures that would damage other tissues such as the skin, muscles, and nerves. With CoolSculpting, a specific body area is precisely cooled to target fat cells in that area. About twenty percent of the fat cells in the treated area die and are eliminated from the body in the weeks following treatment. The patient is prepared for CoolSculpting with the application of a thin film to prevent damage to the skin in the area being treated. An applicator is then applied to the treatment area on top of the film, and in most cases, a vacuum sucks in the treatment area to the applicator. The controlled cooling is applied to the area for one hour in most cases, although some areas require a two-hour treatment. We are born with a certain amount of fat cells, and once they are removed from the body, either through CoolSculpting or an alternative such as Liposuction, they are eliminated forever, so the results are permanent. The benefit with CoolSculpting is that surgery is not required, and there is no recovery period so that patients can return to work or other activities immediately after treatment.


CoolSculpting Applicators

Applicators are just the shape of the body parts and fit easily and will allow, once they are applied for the machine’s suction, to draw the fatty tissue into and maintain a good seal while it’s going through its freezing cycle. Those are the three for the torso that is standard size. Then there’s a large one called Cool Max, a double-sized, large applicator for people with more than the average amount of abdominal tissue. If you have a large gut, you can de-bulk a lot of that fat with treatment using Cool Max and then do fine-tuning and shaping afterward, using the smaller applicators. Then there’s a unique applicator called Cool Smooth, which is used for the outer thighs or the saddlebag area. That’s the only one that does not involve suction because you can’t suck that tissue in. This lies on top of the outer thigh. It straps on and maintains contact with the skin, but because it’s not sucking the tissue into and freezing it near its cooling plates, it has to lay on top, and therefore the treatment time is a little longer when you’re treating outer thighs. Instead of an hour for a typical cooling cycle, it’s two hours for the outer thighs.


CoolSculpting likely be an effective solution?

It is important to consider if the patient will get a good result that they will be happy with. Prospective patients with discrete areas of unwanted fat in areas such as the abdomen, outer thighs, or love handles can be good candidates for CoolSculpting. For the most part, the fat needs to be pinch-able. The other most important factor is that the skin tone needs to be good, without excess skin, which we call laxity in the skin. If these two criteria are met, the patient is likely to be a good candidate for CoolSculpting. A patient considering CoolSculpting should understand what they can expect during the treatment, how long it takes to see the results, and the risks or side effects. Those are the same questions you should ask for any surgical procedure, cosmetic or non-cosmetic, and they apply to everyone. It’s really important to review before and after photos to get a realistic idea of what the results are likely to be. Look at photos on the Internet and your doctor’s database showing patients that they have treated. You should review how patients looked before, after one treatment, and how they looked after two treatments. Look at several patient photo sets to give you an overall realistic idea of the results. Don’t just look at the one best result, which had the most dramatic improvement, because that’s not a fair assessment of what somebody could expect. Asking to see before and after photos for any cosmetic procedure is important to get a realistic view of expectations.


What are some patient types where CoolSculpting would not be the recommended solution?

This is an important question because not everyone is a candidate for CoolSculpting. In my opinion, the ones who are not good candidates are the ones who will not have a very good cosmetic result. That’s the whole reason you’re going to have a procedure; it’s to help shape your body and improve areas where there are unwanted fat deposits. As mentioned before, there must be no excessive amounts of loose skin. People who come in and ask for CoolSculpting of the abdomen but have a lot of loose skin, stretch marks, or separation of the abdominal muscles are not good candidates. Those are perfect candidates for a tummy tuck or an abdominoplasty. They would not be pleased with CoolSculpting results even if they lost a little bit of the fat there as the abdomen’s appearance would still be pretty much the same, and they would be disappointed.
