Pigmentation is mainly cuased dure to irrigular activity of melanocytes, the cells that are responsible for the melanin productions. Melanin is reposible to give the our skin and hair its color. After producing the pigment, the malnocyte reach to the keratinocytes; the cells that form the skin outer layer, and place the pigment in them in forms of melanosomes. One melanocyte is responsible for feeding 30-40 keratinocytes. The color of the skin and hair depends mainly on the type of the melanin and the way it is distributed in the keratinocytes. The Melanin is mainly located within the epidermis, the outer layer of the skin. The presence of the melanin in other layers of the skin such as the dermis is associated with some forms of pigmentation issues such as Melasma.
Melanin is produced in two different pigments: Pheo and Eu. The color of the skin and hair is related to the type of melanin. The Eumelanin is responsible for dark skin colors and the Pheomelanin is responsible for lighter skin and hair colors. The enzyme tyrosinase is responsible for the conversion of tyrosine to melanin by tyrosinase, which is a key step in hyperpigmentation.
Under several conditions, the melanocytes started to overproduce the pigment melanin, and the abundant production results in irregular deposition in the keratocytes, hence the unwanted pigmentation. The type of pigmentation is mainly related to the condition or the calibrate that induced the irregular activity of the melanocytes. For example, melasma is mainly induced by hormonal changes during and after pregnancy, hence the name (mask of pregnancy).
The treatment of unwanted pigmentation and hyperpigmentation mainly depends on three main factors:
1- Fraxel Laser and fractional laser for skin resurfacing to get red of the keratinocytes
2- Very short pulse width Laser and Q-switch Laser to hit the melanocytes that are filled with melanin
3- Silk Peel microdermabrasion with hydroquinon products
